What each measurement means

| mg / mcg | Milligrams and micrograms measure mass. 1 mg = 1,000 mcg. |
| mL | Milliliters measure liquid volume. Volume alone does not tell you the medication amount. |
| mg/mL | Concentration: milligrams in each milliliter. For example, 5 mg/mL means 5 mg in 1 mL. |
| U-100 markings | A U-100 insulin syringe has a scale of 100 units per mL. On that scale, 20 units corresponds to 0.2 mL. This does not mean 20 mg or 20 medication units for another drug. |

From medication amount to volume
Suppose an example amount is 1 mg and the example concentration is 5 mg/mL. These are teaching values, not a suggested dose or preparation.
- 1. Divide amount by concentration.
1 mg ÷ 5 mg/mL = 0.2 mL
- 2. Convert volume to the U-100 scale.
0.2 mL × 100 = 20 U-100 syringe units
- 3. Check the arithmetic in reverse.
0.2 mL × 5 mg/mL = 1 mg
If the concentration changed to 10 mg/mL, the same example 1 mg would occupy 0.1 mL: 10 on the U-100 scale. This is why a syringe number cannot be copied between different concentrations.

How many units are in 1 mg?
There is no universal answer. You need the concentration and the syringe scale. Medication units such as IU are also substance-specific and cannot be treated as a general mg conversion.
Does a 5 mg vial mean 5 mg/mL?
No. A total amount and a concentration are different. A final solution containing 5 mg in 2 mL has a concentration of 2.5 mg/mL. The reconstitution calculator explains this arithmetic using a known amount and final volume; it does not choose a diluent or preparation method.
Can I convert mcg to mg directly?
Yes. Divide mcg by 1,000 to get mg: 300 mcg = 0.3 mg. No concentration is needed for this mass-to-mass conversion. Try the unit converter.
What if a result falls between markings?
A rounded calculator display does not authorize changing your instructed dose. Ask your pharmacist or clinician how to measure it with your actual device, including which syringe and markings to use.
